Include when and where. What are the key findings? What were the root causes? What key learnings were provided? What recommendations were offered to prevent a recurrence? What Happened – provide a brief summary (your original work) of what happened and the consequences of the event. Key Findings – describe the findings of the investigation. Root Causes – identify the root cause(s) and categorize each into relevant categories such as human error, asset integrity, communication, training, competency, etc. Key Learnings – identify the learnings derived from the study that may be relevant to the organization or industry. Recommendations to Prevent Recurrence – identify what recommendation has been generated from the investigation that may be used to prevent a repeat or similar event from occurring in the same or other companies.
