Mentored Experiences (250 word limit)
In this section, describe the mentored experiences that have been valuable to you and that have prepared you to begin a doctoral clinical psychology program. A mentored experience is any situation were an instructor or supervisor provides training. Possible examples include experiences in a class, interacting with an advisor, working as a research assistant, or working in a supervised clinical setting. Describe your mentored experiences in order of their importance to you, beginning with the most important experience.

Research Productivity (250 word limit)
In this section, describe things you have produced relating to psychological research. Possible examples include poster presentations, peer reviewed publications, theses, and research projects or literature review papers for psychology courses. Describe the things you have produced in order of their importance, beginning with the most important.

OPTIONAL: you may attach to your application ONE example of something you have produced relating to psychological research. If you choose to submit an example, include a heading titled, “Research Example.” Under this heading provide a very brief statement with the following information. State the title of the example. State the nature of the example (for example, is it a poster, a paper for a class, or something else). State your role in the example (are you the sole author, or were there co-authors, and if there were other authors, what was your contribution?). State whether, while working on this example, you received supervision, advice, or instruction from someone who is writing a letter of recommendation for you, and if so, state the name of this letter writer.

In this section, describe which clinical faculty member is your preferred research mentor and why. You may select up to 2 clinical faculty members you would like to work with, but only one is required.

Challenging Experience (100 word limit)
In this section, describe a time when you experienced a failure or challenge. Describe the outcome of the situation and describe how you coped during and after the situation.

In this section, describe the attributes of the Baylor PsyD program are most attractive to you. Also describe how your training goals will be met by attending the Baylor PsyD program.

Dr. Sanford’s current work involves investigating health-related attitudes and behaviors and determining ways to increase the types of attitudes and behaviors that are especially important for public health. For example, one significant health problem addressed in his work is the fact that medical treatments often fail to be effective because patients fail to follow their treatment plans. His work also addresses questions such as: why do people sometimes have negative attitudes toward their medical treatment plans, mistrust medical authorities, or fail to maintain a healthy lifestyle with regular exercise? Dr. Sanford’s approach to answering these types of questions includes three key components.
