Thea is a 38-year-old nurse. She has come to see you as she is concerned that she is not coping with an experience that she had. Six weeks ago, she was involved in a traumatic incident whereby the bottle shop she was attending was robbed at gunpoint. At the time of the incident, Thea was standing in the store and was approaching the counter when the offender entered. Thea states that the offender pushed past her, knocking her to the ground. At first, she reported feeling stunned and irritated, wondering what had happened. She stated that she recalls looking up towards the counter and seeing the man who had pushed her holding a gun towards the attendant. She reports that the attendant was very distressed and appeared to freeze at the sight of the offender. The offender, who was already agitated, started yelling even more and became very heightened. Thea reports that there was one other person in the store, a middle-aged woman, who was hiding behind the shelving. Thea reports that she called out to the offender to calm down and asked if she could be allowed to go behind the counter to assist the attendant who was in a clear state of shock. She reports that the offender then turned the gun towards her and started yelling. At that point, a manager, who was in the back of the store, came out and appeared behind the counter. His presence obviously shocked the offender who then fled. Thea reports that following: • Feeling on edge all the time, symptoms of panic • Replaying parts of the experience in her head, constantly assessing what she could have done differently • Images in her head of the offender pointing the weapon at her • Self-criticism in relation to her ‘speaking up’ to the offender. Thea feels that she placed people in unnecessary danger because of this • Loss of appetite • Withdrawal from social networks Thea states that she has dealt with a lot of traumatic incidents at work and has dealt with a lot of things. She discloses that she grew up in a difficult environment whereby there was lots of verbal abuse, yelling and at times physical abuse. Thea states that she does not often think about her past and feels that she managed it quite well. She states that she is confused by her ongoing feelings of stress and feels that this is a situation that she should have been able to deal with. Assessment Tasks: ou will be provided with a scenario that depicts a trauma case presentation. You will be asked to assess the scenario and provide a response to the following: Describe what indicators that you would look for that may suggest that the client is suffering from PTSD (this does not need to be a formal assessment) Develop a treatment plan for your client. What general issues do you face in working with a client with a traumatic history? What might be the specific issues you as a counsellor face in working with a client with this history of trauma? Justify your treatment plan by referring to the literature addressing best practice for a client from this population. Ensure that your treatment plan clearly delineates the assessment and intervention strategies you would undertake. As a counsellor how will you ensure that you at least do no harm? What factors will you need to consider in relation to the therapeutic relationship? Pay attention to integrating the literature and research with the presentation of your specific case and treatment suggestions for your client.
